OUTPATIENT CARE MANAGEMENT

  • Schedule healthcare related appointments
  • Accompany the client to healthcare appointments
  • Be present during patient consultations to make sure all relevant
    information is obtained from the healthcare provider
  • Record Dr’s appointments and make a transcript available to interested parties
  • Advocate for the client's patient rights whenever necessary
  • Make in-home visits to assess and help satisfy the client’s healthcare needs
  • Consult with patient and family to address any questions or concerns

INPATIENT ADVOCACY

  • Make sure that all advanced Directives are on record with the hospital or surgical center
  • Work with designated loved ones and the healthcare team to make sure the patient’s Advanced Directives are carried out when the specified criteria are met
  • Make scheduled hospital visits to make sure that patient’s needs are being addressed
  • Clarify information provided by hospital professionals to the patient and designated loved ones 
  • Consult with patient and family to address any questions or concerns

MEDICATION MANAGEMENT

  • Provide workable and safe methods  for administration of medications
  • Work with healthcare providers and pharmacists to resolve any medication related issues
  • Maintain an up-to-date list of all medications (prescription and over the counter) to be given to healthcare providers when needed
  • Consult with patient and family to address any questions or concerns


LONG-TERM CARE

  • Research Nursing Homes and Assisted Living Residences to provide reviews for family
  • Make scheduled visits to the Nursing Home or Assisted Living Residence to check on your loved one's condition and to monitor the quality of care provided
  • Report on issues regarding the quality of your loved one's care and provide counsel for possible solutions
  • Advocate for your loved one's healthcare rights
  • Consult with patient and family to address any questions or concerns

HEALTH INSURANCE ISSUES

  • Obtain and clarify  information regarding health insurance coverage
  • Act as liaison with insurance company to facilitate claims or resolve issues
  • Consult with patient and family to address any questions or concens
